The use of low-molecular-weight heparin for prosthetic valves remains controversial, but there is agreement that monitoring of factor Xa levels is needed if this agent is used in patients with a prosthetic valve. Low-molecular-weight heparin is not optimal in this patient because anticoagulation may need to be quickly reversed, depending on subsequent therapy.
so that unfractionated heparin is the answer.
