about CCS order

please someone gives your opinion to help me, thanks.
I have a couple of questions about CCS orders
1. how to order "monitoring input/output"?
2. when should we order "complete bed rest" or "bed rest with bathroom privilege"?
3. for pts 40s, 50s, or 60s, if they did not mention anything about alcohol, when you counsel pt, should you order "limit alcohol" or "no alcohol"?
