thyroid problem

ques: a patient got a total thyroidectomy and hase been on levothyroxine since the time of surgery that is 3 years ago. Her imaging showed no metastatis at the time, and she has been adequately followed. NOW,she comes with a very low TSH but higher than normal Total t3 and total t4. But her free t3 and t4 are withing normal range. What is the most likely cause for her lab abnormalities?
