USMLE Step1 covers all the basic sciences including Pathology and Microbiology. Questions tend to be clinical ("wrapped into little vignettes") so knowledge of the typical disease presentations is required too.

There is no minimum score that "guarantees" you a position in any field. The score is only part of the package. I have known IMGs with so-so scores who found their place, and I have seen IMGs with scores in the top 5
percentile who couldn't find the Residency they wanted.

Why do you want to leave the British system? I recommend you graduate from a British school and do the USMLE "on the side" to increase your career opportunities.

You should not have visa problems (once you get accepted into a program, they will sponsor you - either H1 or J1).
