11. A chest x-ray of a 61-year-old man who presented with increasing shortness of breath is found to have a diffuse pulmonary infiltrate. A transbronchial biopsy reveals fibrosis of the walls of the alveoli along with the accumulation of macrophages in the lumen of the alveoli. These macrophages have PAS-positive cytoplasmic granules. What is the best diagnosis?

A. Chemical pneumonitis
B. Desquamative interstitial pneumonitis
C. Hypersensitivity pneumonitis
D. Radiation pneumonitis
E. Usual interstitial pneumonitis
