There are multiple factors which can contribute to insulin resistance.
Adipose tissues secrete certain hormones
1:Adiponectin which increases sensitivity of insulin
2:Resistin which decreases sensitivity of insulin
In obese patients who are prone to DM ,there is increase production of Resistin and decrease production of Adiponetin (One theory)
Secondly ,this is a well known fact the free fatty acids decrease the uptake of glucose in the peripheral tissue.It might be having some role in pathogenesis.
But one thing is sure that
Obesity -----------> insulin resistance---------> type II diabetes.
