In Nephrogenic DI (caused by Li for example), the underlying defect is a lack of ADH working in the body. Which basically means, we have no way of conserving water anymore, which explains the constant urination.
the increase in urination leads to an inability to conentrate the urine which explains why in most DI pts. they have a low urine osmolality and high serum osmolality.
By administering a diuretic (ie: thiazide) we help to increase the urine osm. and decrease serum osm. Its effect to to create equilibrium in the body.
I think the confusing point, is that in normal DI, just cause they piss too much, doesn't imply they pee conentrated urine. The body increases its serum concentration and needs to get rid of it, hence the administration of a diuretic.
