Things that nobody is going to consider: BLS, ACLS, MENSA.
Things that work for you: recent graduate, good LORs, some publications.
Things that work against you: 2nd attempt, not having USMLE certification (you may or may not be certified on time even if you pass the exam this time), unclear if you need a visa.
I think you should have no problems with Family practice; Internal medicine will depend on where you apply as you do not have Internal medicine experience.
You actually may have a chance with General Surgery, but you need to do your research well and figure out what programs accept people with 2 attempts. Another thing to cosider would be a transitional year in Surgery if nothing else works out.
Once again, many programs would not want to deal with visas.
