No, I don't think so. Just tell them how many years of residency you have done. Do not get into details of leaving the residency program. It is like when you disclose a news to a patient, you don't enter into full detail, you do it progressively. You could disuss details in an interview but on your aplication just mention the residency time.
Nobody actually asks you for your certificate of resiudency, there is no such doc on the list of docs to send to eras.
Read the International Medical Graduate's Best Hope sold on amazon for details on how to put together your application to be successful in the match. Scores are important but letters of recommendation are also important along with other aspects of the application.
