It is OK to have fellowship training in some programs without previous US residency experiences, you might have trouble for state license, so you are required to have residency training in the US again. If you just want to have the knowledge, the fellowship training, then back to your country, it is fine. But I think that 99% of foreign med graduate wants to stay in the US to practice.
One exception is that, you are international famous doctor, you do not need to repeat residency and fellowship. I knew a doctor in his 50s at University of Michigan, a Pediatric Nephrologist from Germany, he moved his research lab with lots of funding to the US, he had over 15 people in his lab. He saw patient one day a week. He still needs to take Step 1,2,3, but no need to repeat residency and fellowship, also he is famous and is hired as a professor.
Nobody on this board has this qualification.
