ERAS - LOR querries

Hi, Iam a repeat applicant but Didnt submit any documents nor applied to any programs. Last time I just Took the ERAS token and didnt go ahead. This time I have My step2 CS ready and got certified , So I have few questions regarding LORS

1. How old does the LOR has to be?

2. iam not able to download " Document submission form " from ERAS supporting services. Without that LOR isnt complete. Anyone in simiar situation?

3. Do we need to send LOR and its supporting documents in sealed envelope to ERAS?

4. Iam planning to apply for PTAL but I dont have a SSN right now. Iam gonna be conditional permanent resident in Nov/dec. does CAl med board send recepit of application atleast, hoping they cash $500 cheque?
