when to stop coumadin

Its ok to stop after almost 1 yr if the cause is due to surgery and all other risks had been checked. Right? Pt is non smoker and had been on coumadin almost 1 yr.
guidelines say distal dvt 3 months-
have you heard of chronic coumadin therapy for PE due to surgery-BTW pt wants to stop.
