How to tackle this?

Hi guys, I am working in Radiation Oncology in Europe. After completing my USMLE, I have applied for Oncology residency programmes. I have checked with them. Most say that I need to complete a year in US before applying for PGY2 residencies like Radiation Oncology. Can any one guide me better what to do in these circumstances. For a person like me, doing a preliminary year in US means to move there just for a year and then apply later for a placement in PGY2 which is not gaurantied at all. It will leave me in complete limbo coz if I do not get PGY2 after completion of transitional year in US. In these circumstances I will loose my current job as well in my home country. It feels to me a great obstacle. I have no idea how to tackle it. I do'nt know what others do in similar situations.
Any suggestions ?
