Is DS-2019 just one page?

I received the DS-2019 form from my hospital. It is a one page document and it is not sealed. Is it the way you also received your DS-2019?
I saw in a visa FAQ that the form is a 3 page document, should be sealed that only the immigration officer can open it and then he/she will give you the yellow ( or green) copy.
Is it the old process? or my hospital did something wrong?
