Scores are certainly important, but the interview in my opinion is the most important component to this process. This not only includes the actual interviews but also your dealings with other faculty/staff/residents the rest of the day. It's a good idea to assume that everyone you come into contact with during the interview will have some sort of say. For example, during an interview a couple of weeks back, I sat next to a resident at lunch who I hadn't met and didn't interview with, but as it turned out, his continuity clinic was with the PD. He felt I would be a good fit for the program and would let the PD know as such. So you never know!
Of course, you need to maximize your time during your interview with the PD if you get the chance. You need to be memorable in a good sort of way, and if it feels right, ask about the program's policy on positions outside of the match. I don't think it's always a good idea necessarily to ask directly for a spot, but inquiring more indirectly about their policy in general can get the ball rolling and plant a potential seed. But you definitely need to be in tune with your gut feelings on this one so as not to appear over-zealous or desperate.
Finally, the thank you letter is very important. If you like the program, indicate this in the letter. Oftentimes these letters will go in your file.
