we can start something like, Mr xyz we have excellent supporting programms in our hospital and very effient social service department so if you ever feel like join those(ofcourse depant upon pt disease) please feel free to contact them, i will be happy to give you thier phone no,since the whole idea is to let your pt know that they are not alone and you are thier to help him by offering them help in every which way and also it will be the time where you can by using ur professioal mannerism build pt trust and show ur communication skills by helping them and showing kindness and empathy, hope it will be a little help.
