j 1 research pl advise

iam on j1 research here for 2 years.can any body tell me how can i change my category from j1 research rto clinical?will i have to go back and get visa stamped again or ill have to apply to ecfmg for change in status?what are the chances that ill get the status change request approved?can i get j1 clinical stamped from canada?pl any one iam really desperate-help me out
