To get fellowships - one needs to be ACGME board certified..which in turn needs a residency along with passing boards...which in turn needs USMLE.
There are some short-terms fellowships more like exhange student fellowships that do not give rights to practice. To practice , as far as i know..one must go thru the mormal routes as virtual-usmle said.
Shall find out more from a radiologist friend who came here after his MS in India..
