few questions on cns

1.In usmleworld videoclips, they ask u to always wear gloves while examining oropharynx. Is that true? Do we also need to use light from otoscope to check oropharynx?
2.How to tell the pt. that I'm going to examine the cranial nerves?
3.In another videoclip, doctor checked dull and sharp sensations on both antr and postr aspects of upper limb but only the antr aspect of leg. Does that mean we don't need to do the postr. aspect of leg?
4.Did anybody use the Snellen's chart to check visual acuity?
